This is a case where I would definitely hire a dog trainer. There are lots of ways you can approach dogs and behaviors you can teach to stop the growling / biting, but there might be an overall relationship issue.

For the crate question, I always use treats with dogs that have issues with in and out of the crate. Open the door say "lets go". If he doesn't come out just go sit down and wait. When he comes out, throw a party - get his favorite toy, give him lots of treats and then take him out to potty. Pulling a dog out is never a good idea. Maybe its time to get rid of the crate at night? That would help solve the problem too.

Also, try pretending like you are going to get him out but just give him some food and walk away. The point it is to get him to forget there is an issue.

Not sure if this is going to help at all.
