My dog refuses to leave the yard for a walk or anything else! ?

My 11m doberman refuses to leave the yard for a walk or anything else. It's not the leash because she walks just fine around the house & yard with the leash on but if we try to go out of the fence for walk, etc., she will tug and thrash around until she squirms out of her collar. We have tried different collars and harnesses, we have tried coaxing with treats and toys, but nothing works. We adopted her from the humane society 4 months ago and have tried everything we can think of to get her to go out. We would love to take her on walks, to the park, or just visiting but once we get close to the gate she freaks out. When we have to take her to the vet we have to pick her up and put her in the car and she will shake and cower for the whole ride. Any suggestions? While in the yard she plays great with our other dogs and even the cat but once she leaves her territory she gets upset.
